About the Role
We are seeking experienced HCC (Hierarchical Condition Category) Medical Coders to join our growing remote team. The ideal candidate will have a strong understanding of risk adjustment coding, ICD-10-CM guidelines, and V Codes (Z-Codes), ensuring accuracy and compliance in all coding activities.
You will be responsible for reviewing medical records, identifying relevant diagnoses, and assigning accurate HCC codes to support proper risk adjustment and reimbursement.

Key Responsibilities
• Review medical records to identify all pertinent diagnoses and assign accurate HCC codes.
• Ensure coding accuracy and completeness in accordance with CMS and payer-specific guidelines.
• Maintain a minimum of 95% quality and productivity standards.
• Apply knowledge of ICD-10-CM, HCC models (CMS, HHS-RADV, ACA, Medicare Advantage), and V/Z codes.
• Query providers when documentation is insufficient or unclear.
• Stay updated on changes to risk adjustment guidelines and coding compliance standards.
• Participate in regular audits and feedback sessions to sustain coding excellence.

Required Qualifications
• 3–5 years of hands-on experience in HCC/Risk Adjustment coding.
• Strong understanding of ICD-10-CM, HCC, and V/Z codes.
• AHIMA or AAPC certification required (e.g., CPC, COC, CRC, CCS, CCS-P).
• Demonstrated ability to achieve and maintain 95% quality accuracy.
• Experience coding from various chart types: EMR, PDF, etc.
• Excellent analytical, documentation, and communication skills.

Preferred Qualifications
• Prior experience with Medicare Advantage, ACA Risk Adjustment, or Commercial Risk models.
• Exposure to HEDIS, RADV, or clinical documentation improvement (CDI) processes.
• Proficiency with encoder tools and EHR systems.
